1. Authorization for Testing
I voluntarily consent to the collection and testing of my specimen (which may include urine, saliva, breath, hair, or buccal swab) by ProofLine Testing Group for the purpose requested.
2. Acknowledgment of Procedures
I understand that:
Testing procedures will be conducted in accordance with industry standards
Certain tests may be observed or monitored when required
Refusal to comply with instructions may result in test cancellation or refusal documentation
3. Release of Results
I authorize ProofLine Testing Group and its laboratory partners to release my test results to:
Myself
My employer (if applicable)
Authorized third parties as required for legal, employment, or compliance purposes
4. Laboratory Processing
I understand that:
Some tests are processed on-site (rapid tests)
Some specimens may be sent to certified laboratories for confirmation
Results are based on laboratory findings and not guaranteed outcomes
5. No Medical Advice
I understand that testing services provided are not medical diagnosis or treatment, and I should consult a healthcare provider for medical concerns.
6. Payment & No Refund Policy
I acknowledge that:
Payment is required prior to or at the time of service
Once a specimen has been collected or testing has begun, no refunds will be issued
