Terms of Service

ProofLine Testing Group

Effective Date: March 19th 2026

1. Services Provided

ProofLine Testing Group provides professional testing services including but not limited to drug and alcohol testing (DOT and non-DOT), DNA testing, fingerprinting, background screening coordination, and mobile workplace testing services.

All services are conducted in accordance with applicable federal, state, and local regulations and industry standards.


2. Appointment & Payment Policy

All appointments must be scheduled in advance through our website or directly with our office.

Payment is due at the time of booking or at the time of service unless otherwise agreed upon for employer accounts.

All sales are final. No refunds will be issued once a test has been administered.


3. Mobile Testing Services

Mobile testing services are available for employers, job sites, and group appointments.

Clients are responsible for:

  • Providing a safe and appropriate testing environment

  • Ensuring all individuals are present at the scheduled time

Additional fees may apply for travel, wait time, or rescheduling.


4. Identification Requirements

Valid government-issued identification is required for all testing services. Failure to provide proper identification may result in cancellation of the appointment without refund.


5. Consent & Authorization

By using our services, clients acknowledge and agree that they are voluntarily submitting to testing and authorize ProofLine Testing Group and its laboratory partners to process specimens and release results to authorized parties when applicable.


6. Laboratory Testing & Results

ProofLine Testing Group partners with certified laboratories for confirmatory and legal testing services.

We do not guarantee specific outcomes or results. All results are based solely on laboratory findings.

Turnaround times may vary depending on the type of test performed.


7. Confidentiality

We are committed to protecting client privacy. All personal and test-related information is handled in accordance with applicable privacy laws and industry standards.


8. Limitation of Liability

ProofLine Testing Group is not liable for:

  • Delays in laboratory processing

  • Employer or third-party decisions based on test results

  • Misuse or misinterpretation of results

By using our services, clients agree to release ProofLine Testing Group from liability to the fullest extent permitted by law.


9. Refusal of Service

We reserve the right to refuse service to any individual who:

  • Fails to comply with testing procedures

  • Exhibits disruptive or unsafe behavior

  • Does not meet identification or documentation requirements


10. Changes to Terms

ProofLine Testing Group reserves the right to update or modify these Terms of Service at any time without prior notice.
