By booking and receiving services from ProofLine Testing Group, I acknowledge and agree to the following:
I understand that all specimen collections (including urine, saliva, breath, hair, or DNA samples) are performed following standard procedures. I voluntarily consent to the collection process.
I release ProofLine Testing Group, its employees, agents, and partners from any liability related to:
• Discomfort or minor injury during sample collection
• Delays in testing or laboratory processing
• Test results and how they are used by employers, courts, or third parties
I understand that ProofLine Testing Group does not control laboratory processing times or final results.
I acknowledge that testing services are not medical treatment or diagnosis.
I confirm that I have provided accurate information and valid identification.
By proceeding with booking or testing, I agree to this waiver.
